Features of the manifestation of type 1 diabetes in children in COVID-19 pandemic

Type 1 diabetes is one of the common chronic diseases of children’s age. In recent years there is an increase in the rate of primary morbidity of 1 type diabetes in children with a displacement of peak manifestation for age 5-9 years. Medical History of 110 patients aged from 0.5 to 18 years diagnosed with 1 type diabetes have been analyzed in the Republican Center for Pediatric Endocrinology with a division into comparison group (2018- 2019) and control group (2020). Comparison of the number of patients admitted to the hospital with the first time identified type 1 diabetes haven’t got statistically significant differences, in 2018 - 30, 2019 – 40, 2020 - 39 patients. The duration of complaints before the admission to the hospital was in 2018 - 19.64 ± 15.09 days, 2019 - 31.16 ± 31.04 days, 2020 - 23 ± 16.17 days. T-criterion in all cases less than unit, which indicates statistically not significant differences. Among all patients in 2018 year boys are 56.7%, girls - 43.3%, in 2019 patients were distributed on the sex equally: boys – 50% and girls – 50%, in 2020 girls insignificantly prevailed – 54%, boys – 46% (statistically significant differences in the sex distribution were not found). Among girls the share of children under 10 years old amounted to 70%, among boys - 47%. Significant hyperglycemia was revealed in the hospital: in 2018 – 18.71 ± 6.23 mmol/l, 2019 - 20.88 ± 8.4 mmol/l, 2020 - 22.3 ± 10.45 mmol/l. The level of glycated hemoglobin was higher than the norm in all patients and did not have statistically significant differences (2018 – 11.3 ± 2.09%, 2019 – 11.5 ± 2.78%, 2020 – 11.64 ± 2.54%). During the period 2018-2019 a correlation dependence was revealed between the length of complaints and the level of the glycated hemoglobin (Person’s criterion = 0.408) and the level of glycated hemoglobin and age (Person’s criterion = 0.276, correlation 95%). In 2018 36% of children were in a state of laboratory and clinical acidosis, which is confirmed by the data of the acid-base state of the blood, in 2019 - 32%, in 2020 - 25%, statistically significant differences were not found. During COVID-19 pandemic the age displacement of the peak of manifestation of type 1 diabetes is preserved for the age of pre-pubertal, more pronounced in girls. There is a correlation dependence between the level of glycated hemoglobin and the age of patients, the duration of complaints.
