Delayed puberty by anomalies hypo-gonadotropic hypogonadism, hyper-gonadotropic hypogonadism and puberty induction

Introduction: Delayed puberty is defined as the absence of the development of sexual characteristics beyond the age of 14 for boys and 13 for girls. To better understand this pathology, we conducted a retrospective study over a period of 7 years; between 2013 and 2020, involving 11 patients with a delayed puberty, followed by a pediatric endocrinology consultation at the children’s hospital Ibn Sina in Rabat.

Patients: Our patients are divided into 18.2% of girls and 81.8% of boys with a sex ratio of 4.5 (Boys/Girls). The average age for recognizing a delayed puberty is 14 years for girls and 15 years for boys. Statural delay is found in 72.7% of patients with a delayed puberty. The latter is severe in 18.1% of the cases (-4DS). Complementary examinations carried out for our patients led to the following diagnoses: Simple pubertal delay in 3 patients ; 3 patients have hyper-gonadotropic hypogonadism ; 5 patients présentent un hypogonadisme hypo-gonadotrope. 5 patients were treated with sexual androgens and 3 of them showed a marked improvement in puberty.

Conclusion: Delayed puberty is a multi-factorial process whose diagnosis is based, firstly on the clinic but also on the realization of the hormonal assessment, the bone age, as well as other biological and radiological examinations. Moreover, the pubertal induction by sexual androgens is the cornerstone of the treatment of delayed puberty.
