Did Covid-19 pandemic restrictions impact on diabetic care of children and adolescents in Armenia?

Background: Type 1 diabetes mellitus (T1DM) is an autoimmune disorder, which occurs mostly among children and adolescents. Armenia is a middle-low income country, where pediatric diabetic care is given centralized in three main centers. The 85-90% of diabetic children and adolescents are followed in the university hospital. During Covid-19 pandemic Armenia was under either lockdown or strict restrictions. The purpose of the current study is to assess the dynamics of mean glycosylated hemoglobin (HbA1c) levels in T1DM children and adolescents before, during and after the Covid-19 restriction period.

Materials and Methods: HbA1c levels; sex and age of 250 randomly chosen T1DM patients were evaluated from 2018 until 2021 years. The patients were divided into 4 age groups: I (0-4years); II (5-8years); III (9-13years) and IV (14-18years). All the patients were on multiple daily injections (MDI) regimen. Statistical analyses were performed with SPSS Version 21.0 to determine the significance of findings. In all cases null hypothesis was rejected if P<0.05.

Results: A total of 250 patients of were included in the study; about 48.8% were females. It is shown in the Table 1, that there is a connection between age and HbA1c levels, HbA1c is higher in older patients without statistical significance (P>0.05). Mean HbA1c is higher in female comparing with male not significantly (P>0.05). Based on the results, there is an increase in HbA1c mean level during 2020, when the pandemic restrictions in the country were lounged (P<0.05), particularly the tremendous increase was observed in the age groups II-IV.

Conclusion: T1DM children and adolescents had higher mean HbA1c levels in 2020, when Covid-19 pandemic restrictions were implemented. Tele-medicine improvement and implementation in Armenia could be an option to improve the follow-up of T1DM children in lockdown or restriction periods leading to normalization of HbA1c levels as a goal.
