4D-Blood+ Proteomic Profiling of Obesity and Metabolic Syndrome in Adolescents

Objective: Obesity accounts for 8.9% of children and adolescents in China, and metabolic syndrome (MetS) accounts for 28.8% of obese children, rising year by year. There is no early prediction and screening index in the clinic, so it is urgent to develop early prediction of MetS in children and adolescents and screen pathogenic molecular markers. Therefore, we propose to establish and screen biomarkers for the prognosis of obesity in children and adolescents through serum 4D proteomics and metabolomics.

Methods: 10 obese patients (OB),10 obese patients with metabolic syndrome (MetS), and 10 healthy controls (Con), all aged 10-14 years, were selected for serum 4D-BLOOD + high-throughput proteomics study to reveal the changes of serum proteome related to obesity and metabolic syndrome in adolescents.

Results: Serum proteomics showed that 15 KGEE enrichment signaling pathways were changed in the adolescent simple obesity group, including progesterone mediated oocyte maturation, Rap1 signaling pathway, platelet activation, insulin resistance, etc. Twenty-six signaling pathways were changed in the obesity combined with MetS group, of which seven pathways were changed in both OB and MetS. However, most of the changes in signaling pathways in the MetS group were related to the activation of neurodegenerative diseases and cancer pathways, and these pathways were normal in the OB group.

Conclusion: It is speculated that activation of neurodegenerative diseases and cancer pathways and down-regulation of inflammatory signaling pathway are closely related to numerous clinical manifestations and complications of MetS in children and adolescents, and that Rap1 signaling pathway, activation of platelets, and down-regulation of inflammatory signaling pathway are also speculated in the pathogenesis of MetS. Neurodegenerative diseases and activation of cancer pathways are important causes of blood proteome changes and related complications. This study provided a multiomics basis for the development of a prognostic test for adolescent obesity, so as to identify high-risk patients as early as possible and trigger early intensive surveillance and intervention.
