Design, certification and evaluation of an Insulin Infusion Therapy Care and Education Program (PAETII) in children/adolescents in a Pediatric Unit

A structured program of therapeutic education is essential to achieve a good result from the use of technology in the treatment of diabetes.

Objectives: Facilitate knowledge, skills and aptitudes to patients and their families in therapy. Promote the capacity for self-management and adherence to treatment. Provide ongoing support to patients. Improve clinical outcomes, health status and quality of life.

Methodology: In 2019, the multidisciplinary work team made up of pediatricians, endocrinologists and nurses was created. For a year, the PAETII program was developed based on the quality characteristics of the Standard: “Educational program for patients who start therapy with the Subcutaneous Insulin Infusor, ISCI”. The program was planned in 3 parts: structure, educational process and results. Material and human resources were taken into account in the structure: training room, number of sessions and participants per session, schedules and duration, educational techniques and materials, instruments for evaluation, records and number and training of professionals. In the process, the structured therapeutic education program was developed, which consisted of an initial assessment of the patient and an agreement of objectives with the patient/family, eight training sessions distributed in three modules (Previous CSII, CSCI start and CSCI follow-up) and a final stage with three patient control consultations. For the evaluation of results, 20 quality indicators were defined that will be evaluated annually.

Results: The process map that described how the program worked was defined. In January 2021, the program was launched and in June, the DNV auditor carried out the first external audit, issuing the Declaration of Conformity. During the year 2021, nineteen children/adolescents and their families participated in the program in one of the five planned editions. The results of the annual evaluation are collected in the analysis of indicators, the most significant being: coverage rate 49%, dropout rate 0%, compliance with scheduled visits 97.6%, percentage of patients trained after completing the educational program 100 %, degree of patient satisfaction 9.4.

Conclusions: The annual evaluation of the program has been very good. Incidents and improvements have been detected that have made it possible to correct some aspects of the program. Indicators are being worked on to measure health outcomes for children/adolescents.
