An extraordinary case, Leydig cell tumor in a 6-year-old girl with significantly elevated 17-OH Progesterone

We report a case of a 6-year-old girl who presented to our clinic with virilizing symptoms (clitoromegaly, acne and pubic/axillary hair), in addition to elevated androgen levels. She was firstly misdiagnosed as a case of non-classical congenital adrenal hyperplasia; due to the severely elevated 17-hydroxyprogesterone (17-OHP) level. Thereafter, with progressive clinical and biochemical hyperandrogenism despite adequate treatment with hydrocortisone, further evaluation was done to reveal a right ovarian mass, which was not seen on the initial imaging and was confirmed to be a Leydig cell tumor after successful laparoscopic salpingo-oophorectomy had been done. Subsequently, all androgenic levels were normalized.
