Adipocytokine status and vitamin D level in large for gestational age newborns

Objective: to study the features of the adipocytokine status and vitamin D status in large for gestational age (LGA) newborns.

Materials and Methods: 348 full-term newborns were examined. Three groups of LGA newborns were identified: taking into account the presence of mother’s type 1 diabetes mellitus (DM) (Gr1, n=58, body weight (BW) – 4225 [4000; 4580] g), excess BW or obesity (Gr3, n=65, BW 4300 [4200; 4540] g), absence of carbohydrate and fat metabolism disorders (Gr2, n=66, BW 4185 [4100; 4320] g). Group 4 (Gr4, n=159, BW 3380 [3170; 3650] g) – appropriate for gestational age newborns from mothers without changes in carbohydrate and lipid metabolism.

Results: The mothers of the examined groups’ newborns had statistically significant differences in the content of leptin (P=0.006), adiponectin (P<0.001). Serum leptin was higher in the group of women with overweight and obesity (32.0 [24.0; 48.0] vs 13.0 [6.0; 21.0] ng/ml in Gr1, 8.0 [4.0; 8.5] ng/ml in Gr2, 10.0 [6.0; 18.0] ng/ml in Gr4, p2-3=0.005, p3-4=0.027). The level of visfatin in Gr3 mothers was lower than that of Gr4 (8.6 [7.8; 10.0] and 12.0 [9.4; 27.0] ng/ml, P=0.020). Content of 25(OH)D in Gr1 was 13.1 [10.0; 17.7] ng/ml, Gr2 – 14.9 [11.6; 23.6], Gr3 – 10.9 [9.0; 16.8], Gr4 – 13.8 [9.3; 20.2] ng/ml, P=0.504. Significant differences in the content of leptin (P<0.001), adiponectin (P=0.006), IGF-1 (P=0.024), 25(OH)D (P=0.005) were found in newborns at birth. Adiponectin and IGF-1 concentrations in Gr1 were significantly lower than Gr2 (P=0.004, P=0.031). Vitamin D was in the lower normal range in all groups. The lowest level was 25(OH)D in newborns from mothers Gr3 (11.0 [8.0; 17.8] ng/ml, p3-4=0.003). Gr3 were divided into 2 subgroups: Gr3a – newborns from mothers with obesity and gestational DM (n=20), Gr3b –from mothers with overweight or obesity without DM (n=45). In Gr3a the levels of leptin and visfatin significantly exceeded those of Gr3b (19.8 [15.9; 37.4] vs 12.4 [7.2; 21.1] ng/ml, P=0.007 and 19.5 [14.5; 62.0] vs 10.3 [9.0; 13.0] ng/ml, P=0.034). The content of 25(OH)D in Gr3a was 17.4 [12.4; 28.7], Gr3b – 10.3 [7.9; 14.6] ng/ml, U=135.5, P=0.046.

Conclusion: The influence of women’ metabolic disorders on the parameters of hormonal status and vitamin D status in LGA newborns was more intense if the mother has overweight or obesity.
